1. The promoter of this prestigious award “Dr. APJ Abdul Kalam Memorial Excellence Awards” is “MOTHER INDIA CARE”, a public charitable, irrevocable, not for profit registered trust, having its registered office at No. 418 & 419, 4^th Floor, B-Wing, Mittal Tower, M.G. Road, Bangalore-560001.

2. Method of Nomination

You can download the nomination form from the website and send the completed form to the above mentioned registered address of the organization/Trust.

Online:
You can go to the website at http://www.motherindiacare.com/submit_nomination_form and fill in the requisite details to complete the online nomination form and submit.

3. Start date for nomination entries: 01 Jan 2018

4. Closing date for entries: 23:59 on 15^th  Oct 2018

5. Shortlisted Nominees notified: By 20^th Oct 2018, on 20th Nov wildcard nominations for award can be accepted only for 2 persons.

6. Winners announced: Excellence Award Event ceremony session on 18^th November 2018.

7. Eligibility:

Nominees (“Nominees”) must be Indian residents, employees (including their families), any of the category sponsors, anyone materially connected to the administration of the Awards.

8. There is no limit on the number of nominations you may submit.

9. You can nominate yourself or someone else but it is your responsibility to obtain permission from the person you are nominating before submitting the nomination.

10. If you are under eighteen (18) years old, you must have the permission of your parent or legal guardian to submit a nomination and you must provide the name and mobile phone number of your parent or legal guardian on request.

11. Nominees must be available to be contacted between 20^th October and 10^th November 2018.

12. The shortlisted and winning Nominees must agree to take part in any interviews, photographic shoots and post-winning publicity surrounding the Awards and will be required to give consent for MOTHER INDIA CARE and selected partners to use the images and interviews howsoever they see fit, including for promotional purposes, with no financial claim by the winner. It is your responsibility to make the Nominee aware of this.

13. As part of the selection process, Nominees may be asked to disclose whether they have ever been convicted of a crime or are the subject of on-going criminal proceedings or investigations.

Proof of eligibility must be provided upon request.

14. Nominations submitted online using methods generated by a script, macro or the use of automated devices will be void. Nominations that are incomplete, illegible or indecipherable will not be valid and will be deemed void. Use of a false name or address will result in disqualification.

No nominations or photographs can be returned.

Prizes:

15. Each shortlisted Nominee will receive two (2) entry passes to the Awards venue.

16. The shortlisted Nominees and their guests are responsible for expenses and arrangements not specifically included in the awards invitation card.

17. The Prizes are as stated and are not transferable to another individual. No cash or other alternatives will be offered. The Prizes cannot be used in conjunction with any other special offer.

18. Awards are subject to availability. MOTHER INDIA CARE accept no responsibility whatsoever for elements of the awards being withdrawn. In the event of an awards Prize being unavailable, the MOTHER INDIA CARE reserve the right to offer an alternative prize of equal or greater value.

19. The Prizes are subject to the terms and conditions of the relevant sponsors or prize supplier which will be provided to the Winner when booking the Prize or with the Prize (as applicable).

20. The Winners in each category will receive their awards at the Awards ceremony on 18^th November 2018. There will not be any monetary or similar prize. If the Winner is not able to attend the ceremony, he/she should intimate us in and advance.

21. MOTHER INDIA CARE will send the award to them.

22. MOTHER INDIA CARE reserves the right to refuse entry to the Awards ceremony to any individual or group.

 Nomination Selection Process

23. All nominations received by the closing date and time will be considered.

24.  The shortlisted Nominees will be selected by the judging panel from the best citation and examples of excellence and the judging panel will be influenced by the number of nominations for any particular nominee as well as the strength of their entry.

25. The judging panel consists of members of the Mother India Care team and at least one independent judge, who will decide the shortlist. The decisions of the judging panel will be final and no correspondence will be entered into over the decisions.

26. Judging of nominations will take place from 15^th to 20^th October 2018 and shortlisted Nominees will be notified by 20^h  Oct 2018.

27. Shortlisted Nominees will receive invitation card by 25^th October 2018. The Winners will be announced at the Awards ceremony on 18^th November 2018.

28.  Failure to respond to information requested by MOTHER INDIA CARE, failure to provide a valid Indian address for delivery, or failure to meet the eligibility requirements may result in forfeiture of the award/Prize.

Use of information provided

29. All nominations will become the property of Mother India Care and promoters accept no responsibility for nominations unsuccessfully submitted.

30. By submitting a nomination form, your consent to give MOTHER INDIA CARE a royalty-free, non-exclusive license to use, reproduce, modify, publish, create derivative works from, and display the nomination (and any photograph included with the nomination) in whole or in part, on a worldwide basis, and to incorporate it singly or collectively into other works in any form, media or technology now known or later developed (including, without limitation, magazines and online), including for promotional or marketing purposes. If necessary, you will sign any necessary documentation that may be required for MOTHER INDIA CARE to make use of the non-exclusive rights you are granting to use the nomination (and any photograph included with the nomination).

31. MOTHER INDIA CARE reserve the right to make the nomination and all reproductions thereof available, either wholly or in part, in any manner or form whatsoever and in any medium or format, either separately or in conjunction with any other content.

32. MOTHER INDIA CARE reserve the right to publish names, photographs and country of residence of any Winner. The Winner agrees to the use of their name, photograph and disclosure of county of residence.

33. You agree to waive all moral rights or any similar personal rights in any nomination you submit.

34. Any personal information, including without limitation, your and the Nominee’s name, age, address (including postcode) and/or email address will be used by MOTHER INDIA CARE in accordance with its privacy policy available at www.www.motherindiacare.com.

35. Your information and the information of the Nominee may be shared with the relevant class sponsor and event management company Events Club India for the purposes of prize fulfillment and the administration of the Awards, which will use such information subject to its privacy policy.

General

36. By submitting a nomination you agree to be bound by these terms and conditions.

37. Your submission of nominations is not dependent on any purchase.

38. No responsibility can be accepted for entries lost, damaged or delayed in the post, or due to computer error in transit.

39. MOTHER INDIA CARE reserve the right to hold void, suspend, cancel or amend the Awards at any time. MOTHER INDIA CARE reserve the right to change these terms and conditions at any time.

40.  MOTHER INDIA CARE also reserve the right to cancel individual nominations without liability to affected participants where such nominations do not (in the opinion of MOTHER INDIA CARE) enter into the spirit of the Awards or if the security or proper administration of the Awards or the validity of a nomination is adversely affected. In the event of cancellation, termination or suspension,

41.  MOTHER INDIA CARE may, at their discretion, select a winner from eligible, non-suspect or unaffected nominations, received prior to the event requiring such cancellation, termination or suspension.

42. In the event of a discrepancy between these terms and conditions and the details in the promotional material, the details in these terms and conditions shall prevail.

43. These terms and conditions shall be governed by the laws of Union of India and any dispute shall be subject to the exclusive jurisdiction of the Bangalore city courts.
